Vaka raporu, genellikle bir veya daha fazla hastanın yaşadığı teşhis veya tedavi sorununu gösteren ayrıntılı bir anlatımdır. Vaka raporları genellikle yeni müdahaleler için ilk kanıt olarak hizmet eder veya halihazırda uygulama alanı bulmuş bir tedavide bir sorunun var olduğuna dair alarm işlevi görür. "Vaka kaydı" (case record) terimi çeşitli yönlerden yaygın olarak kullanılırken, "vaka raporu" (case report) terimi yalnızca modern tıp dergilerinde kullanılmaktadır. Vaka raporu, belirli bir klinik kanıt biçimi olarak nadir veya alışılmadık hastalıkları tanımlamada; tedavilerin etkinliğini, güvenliğini ve maliyetini değerlendirmede; ve tıbbi eğitimi, hasta bakımını ve tıbbi araştırmayı iyileştirmede önemli rollere sahiptir. Vaka raporlarında sunulan değerli bilgiler ve yeni fikirler sonucunda yürütülen önemli çalışmalar, araştırmalar ve klinik uygulamada kaydedilen ilerlemeler sonucunda hasta sonuçlarında iyileştirmeler sağlanabilir ve bu raporlar günümüzde birçok dergi tarafından yayınlanmaktadır. Vaka raporlarının yayınlanması tıp literatüründe bir standart haline gelmiştir.
